Lxanvom and the Twelve Kings
The Crypt of Decay in the Plane of Disease began on Antonica as Lxanvom, the Kings Rest of a long-dead race. A priest of decay sacrificed its last king there and bound twelve royal spirits to Bertoxxulous, and their plague wiped the race out.

Kings Rest
The ninth part of the official Planes of Power lore series says the crypt was first built on Antonica, in an age long gone, to hold the dead kings of an ancient race. In their tongue its name, Lxanvom, meant Kings Rest.
As the race filled it with dead royalty, a festering evil took hold in the crypt’s lowest depths. Some scholars call this the birth of Bertoxxulous; others say he has existed since time began and this is merely the oldest record of his coming.

Ultor Szanvon the Putrid
The first priests of decay were led by Ultor Szanvon the Putrid, who schemed and murdered his way up to chief advisor of the reigning king. He lured the king into the crypt, sacrificed him in a dark ritual at its center, and called up twelve spirits of earlier kings, binding them to his service.
The twelve carried a deadly plague, which spread like wildfire and destroyed what was left of the ancient civilization. Bertoxxulous then pulled the whole crypt into his home in the Plane of Disease. The form he is said to prefer when he visits it is a twisted version of the race he wiped out ages ago.

The twelve heroes
Each of the twelve was given a form of its own, except the four oldest, who share one. The article’s profiles name them all with the surname Adan.
Darwol, the last king given a private burial chamber, was forced into the shape of a ravenous rat and bears the weapon Dark Corruption. Feig, the eleventh king, whose people honored him with his own holiday, made the Putrefied Belt of Death. Xu, a sorcerer king, is credited with the Wrath of Souls. Kavilis, the Scholar King, who led his people in building the first schools and universities, became a twisted, malevolent mosquito.
Raddi, a warrior who held off thousands of troops, became an undead knight with the blade Blood Defiance. Wavadozzik, the seventh king, thought by some the wisest of them, became a spider creature. Zandal, a noble king, became a fly. Akkapan, known as the Nimble, became a quick-footed rat with the daggers Green Tears and Red Tears.
Meedo, the fourth king, remembered for rebuilding the royal palace, and Qezzin, the third, who made a great treaty with many neighboring kingdoms, both became hulking diseased humanoids. So did Pzo, a warrior monk who led a great enlightenment across his kingdom and wields the Fist of Death. Bhaly, the Founder, made the legendary Torch of Lxanvom, part of a key to the Crypt of Decay; his own form is not given.

Where the article disagrees with itself
The article is not consistent. Its summary sentence promises twelve kings but lists eleven, with different surnames from the profiles, among them Feig Glaemod, Xu Ezuqu, Kavilis Dri, Qezzin Adan and Bhaly Nestif, and it leaves out Darwol. It also says the four oldest share a form, while the profiles give that hulking form to the third and fourth kings and to Pzo. Only the eleventh, seventh, fourth, third and first kings are numbered.

The Torch of Lxanvom
The tenth part of the series says the torch was placed deep in the burial tomb Bhaly built for himself and his ancestors. It was used in the burial rite performed on each king before he was laid to rest, sealing him away in everlasting peace.
Nobody has known where it is since the civilization fell. It is said to look like an open torch burning with a blue flame.
