The Plane of Disease
The Plane of Disease is the realm of Bertoxxulous, the Plaguebringer: a land of rotting flesh and rivers of pus. Past a ridge of bone lies his temple, and within the plane is the Crypt of Decay where he lives.

The land
The fourth part of the official Planes of Power lore series says the plane is ruled by Bertoxxulous. Its ground is diseased, rotten, putrid flesh, and rivers of pus run across it to form a swamp of bile. When it rains, green bile falls from the sky.
A narrow pass cuts through a mountain ridge of bone to the high ground in the north, and beyond that ridge lies the Temple of Bertoxxulous. The plane also holds the Crypt of Decay, where the god himself lives; the article names both places without saying whether they are one.

The swamp and the Wanderlands
Grimror’s Swamp of Bile is a wide area full of flymen and malarians. The flymen are quick to spit deadly venom that carries many diseases. The malarians near the rivers are known for their aggression and for a venom that blinds their prey.
Spiders live in the Wanderlands in the west of the plane. They first lived in the fleshy caverns, but many have since moved out onto the surface. They share the caves with the bubonians, rat-like creatures that carry a host of diseases.

The Keep and its guards
The Keep surrounds the Crypt of Decay. The Lepertoloth guard it, magical beings who keep unwanted visitors out of the crypt. Gryme carries the key to the Keep.
Grummus was made by Bertoxxulous to guard the entrance to his den, and carries the ward needed to enter the Crypt of Decay.

Grimror Plaguebringer
Grimror Plaguebringer is an ogre shaman who has chosen to stay on the plane. He follows Innoruuk, not Bertoxxulous, and his task is to brew vile new plagues and diseases to loose on Norrath.

Where the bubonians came from
A later part of the same series tells how the bubonians began. Tarkon Praez, a necromancer serving Bertoxxulous, performed a ritual that called for a unicorn’s heart, a phoenix’s tail, a virgin’s tears and a faerie’s blood. It produced a deeply evil, crazed race of rat men who carried a foul plague.
A good and noble order of knights drove them out, and Bertoxxulous then took the bubonians into the Crypt of Decay.

The Reign of Decay
The plague did not stay at home. The series says that during the Reign of Decay, Bertoxxulous sent a single tsetsian fly into the Plane of Valor, and its larvae turned some of that plane’s people undead. See the Plane of Valor for the plane it reached.
